Are ear pain that doesn't resolve with treatments recommended by an ENT doctor? Do you feel pressure in your ears, ringing, or dizziness, but investigations show everything is „normal”?
The truth is that very few symptoms considered „ENT-related” can have a hidden cause: Temporomandibular joint (TMJ).
What is the temporomandibular joint (TMJ)?
The temporomandibular joint is Temporomandibular joint, located just in front of the ear.
It is a complex joint, formed by mandibular condyle, articular disc, and temporal cavity.
It is surrounded by ligaments, muscles, and is traversed by branches of the trigeminal nerve, which is also responsible for facial sensation.
The TMJ is the „hinge” that allows us to open and close our mouths, chew, and talk.
Because it is located very close to the ear and cranial nerves, dysfunction ATM can cause symptoms sounds like ENT problems
ENT symptoms that can actually be caused by TMJ dysfunction
Patients with TMD often present to the ENT doctor because they experience:
Ear pain (otalgia) – appears without infection, without earwax blockage, without visible inflammation.
Feeling of a blocked ear – many patients describe a constant pressure, as if they „had water in their ear”, even though the eardrum is normal.
Tinnitus (ringing in the ears) – Persistent ringing, sometimes throbbing, dull, or sharp, with no identifiable cause on the audiogram.
Dizziness and vertigo – TMJ dysfunction can unbalance posture and influences the vestibular system.
Retrauricular pain or pain in the temple area – in fact, irradiation of jaw joint pain after contractions of the masticatory muscles.
Temporary hearing loss – a fluctuating sense of hearing loss, without inner ear damage.
These symptoms are often labeled as „functional” or „unexplained” ENT issues, when in reality, they stem from a jaw problem.
Why can the TMJ cause ear symptoms? (Simple anatomical explanation)
Anatomical proximity The ATM is a few millimeters from the external auditory canal. Inflammation or pressure in the joint is felt directly in the ear.
Trigeminal nerve – branches that innervate both the TMJ and structures of the ear. Jaw irritation can be perceived as „ear pain.”.
Muscle Chain – the muscles that move the jaw are connected to those in the inner ear and neck area. Contracts can create feelings of pressure and noise.
Joint pain – if it is displaced or inflamed, it can press on neighboring structures and create a feeling of blockage or clicking in the ear.
⚠️ The difference between actual ENT problems and those caused by the TMJ
Symptom | ENT cause | ATM Cause |
Ear pain | Infection, otitis, earwax plug | TMJ dysfunction, muscle tension |
Muffled ear | Middle ear fluid, deviated septum | Joint compression, mandibular imbalance |
Tinnitus | Noise exposure, sensorineural hearing loss | TMJ imbalance, muscle contractures |
Dizziness, Vertigo | Meniere's disease, labyrinthitis | ATM disorders + cervical posture |
How do we investigate at Innovation Medical Center?
To make The clear difference between ENT and TMJ, we use a unique protocol:
Consult interdisciplinarilyMedic dentist, specialist in physical medicine and rehabilitation, specialist in oro-maxillofacial surgery.
TMJ Ultrasound – visualize the joint structure.
Pupillometry and/or InnoByte – check the connection between ATM and the nervous system.
Thermography – shows invisible muscle tension zones.
Postural assessment with AI – identify muscular imbalances that perpetuate symptoms.
Treatment of Temporomandibular Joint (TMJ) Dysfunction with ENT Symptoms
The treatment depends on the cause and is personalized. In our center we use:
Photobiomodulation Laser (LLLT/HILT) – reduce inflammation and pain.
Custom neuromuscular splints – Repositions the mandible functionally correctly, eliminating pressure on the joint.
Postural therapy – corrects muscle imbalances that perpetuate symptoms.
Laser acupuncture – adjusts nervous and muscular tensions.
The result? „Unexplained” earaches disappear, ringing decreases, and balance and quality of life improve.
Frequently Asked Questions
Ear pain without infection, earwax, or other ENT causes can stem from the temporomandibular joint (TMJ). The trigeminal nerve, which innervates the jaw, also transmits pain to the ear area.
Yes. Patients often describe a feeling of pressure or „water in the ear” even if the eardrum is normal. This happens because the TMJ joint is located very close to the ear canal and can irritate the structures in the area.
Tinnitus can occur when the muscles around the TMJ are tense or when the joint disc is displaced. These changes can alter nerve transmission and generate ringing in the ears.
If you have recurring ear pain, but ENT treatments aren't working and investigations come back normal, it's recommended to get a TMJ evaluation. Other associated symptoms can include: jaw clicking, headaches, jaw locking, or neck tension.
The treatment includes laser photobiomodulation for pain reduction, custom splints for jaw repositioning, physical therapy and postural therapy, as well as laser acupuncture. The goal is to eliminate the cause, not just mask the symptoms.

Conclusion
If you have ear pain, clogged ear sensation, tinnitus, or dizziness, and the ENT doctor tells you there's no obvious cause, the problem could be at the temporomandibular joint level.
